Xray Forms The xray form is one form the students will use frequently. Any xray, CT, or U/S must have a form filled out to give a Dx and indication for the xray. |
ED Order Forms The ED order form is where you will place all orders for labs, xrays, and phone calls. You may fill this form out after you review your cases with your supervising physician. |
